- Check skin. Document any changes (redness, cracks, fissures).
- Apply or to dry, cracked, or chafed skin. Use small amounts and massage gently and thoroughly into skin.
- Do not massage stage 1 pressure ulcers (nonblanching hyperemia). This avoids additional trauma to compromised skin.
- Do not apply between toes or in skin folds, where moisture may get trapped and cause maceration.
- Educate resident/caregiver on the importance of fluid intake and the need to reapply a skin conditioner/moisturizing cream daily.
- Discard disposable items in a plastic trash bag and secure.
- Document findings, and update care plan.
